Dear Sports Information Director:
On Friday, January 28, 2005, the 18th Annual Woody Hayes National Scholar-Athlete Awards will be presented by the University and Northwest Sertoma Club of Columbus, Ohio. These awards are given to the outstanding man and woman in each of the three collegiate divisions for demonstrated excellence in athletics, academics and community service.
Enclosed is additional information about the awards and your nomination form for this year’s awards. Several important points:
· The nomination deadline is December 17, 2004, and you may send the nominations by mail, fax or e-mail.
· Winners must have completed their eligibility in calendar year 2004. For example, athletes who completed their basketball in 2003-2004 are eligible. 2003-2004 basketball players would not be eligible.

These awards are presented to the outstanding male and female scholar-athlete in each of the three collegiate divisions. A panel of eminently qualified judges select the awardees.
